Volunteer Request Form

For the safety of all volunteers and participants, we ask every individual who participates in any of our large events to sign a waiver acknowledging all of the possible risks and dangers, surrendering the right to sue, and giving full rights to AYSC on handling any emergency situations. For minor volunteers, parents or guardians will be responsible for signing the waiver.
